2021-2022 School Year Annual Registration Information

Parents/Guardians,
As you know, at the beginning of every school year parents/guardians are responsible for filling out a registration packet for each student. All of the forms you are asked to fill out may seem redundant, but they are required by the state. This information is used for safety, health and communication purposes as well as factoring into our funding.

This year, DWCS has prepared an annual registration packet that can be downloaded as a fillable .pdf file. It is the same forms that we always have, but you will be able to download the forms, type in the information, save the file and email it to annual@darbonnewoods.com. Or you can print the forms out, fill them out by hand and drop them at the high school office Monday-Thursday, 8 a.m. – 11:30 a.m. or 12:00 p.m. – 3 p.m. You are also welcome to mail them to DWCS Annual Registration, 9560 Hwy 33, Farmerville, LA 71241. If you are unable to download the forms, you can pick up the packet at the high school office as outlined above.

If you choose to fill the registration forms out on your computer, the forms will auto-populate certain information such as the student’s name, grade, mailing and physical addresses, etc. that are asked on more than one of the forms. Just remember that you will need to print out the .pdf with your answers or save the .pdf with changes to your computer so you can email it. If you don’t save the form with changes you will lose your typed information.

Fill out the forms as completely as possible. If something doesn’t apply to your student, enter “N/A” so we know that the question wasn’t overlooked.

We are requiring registration packets to be turned in by August 13th this year so that your student’s information can be distributed to the departments that need it and updated before Parent Orientation. Our goal is to make it possible for parents/guardians to have all of their back-to-school responsibilities taken care of before our students step back on campus.

ITEMS THAT MUST BE TURNED IN BEFORE YOUR STUDENT ATTENDS CLASSES:
Signed Demographics form confirming contact information (you will get this from the student’s homeroom teacher at Orientation), Up-to-date Immunization Records, Free/Reduced School Meals Application, Health Plan, medications, physician’s forms as required by the school nurse.
